The client’s name is John Smith. John is six years old biracial male, born on December 19th, 2011. John has attended church for a month to month basis, but it is unclear if he has a preferred religion at this time. His ethnicity is mixed with African American and White.
He currently lives with his single mother, but due to many years of ‘roller coaster’ events, his behavior has been shown signs of depression are beginning to show. In his classroom he does not focus, he has not respected for teachers, classmates or other adults there. Though, he listens to his grandmother and adults outside of school. He avoids confrontations and uncomfortable situations.

The referring source is John’s first-grade teacher, Ms. Ginger and principal Mr. Wonder who have been observing John along with a child specialist. John’s referral is concerned with the way he is interacting with his fellow classmates, teachers, as well as his class and homework was being not done. John is unaware that he will be starting a weekly session with a social worker, though he is voluntarily going and he is aware that he is going to see one. The client is aware of why he is going to see a social worker. John knows that his actions with his classmates are getting out of hand. Jones, it is obvious that without the help of the mother, grandmother, babysitter, and teachers, John will have no improvement in his behavior. Since John has multiple influences in his life, multiple adults who are raising him, he is confused on how to incorporate all of the rules into his own life. For the past few years since Ms. Jones has started to work three jobs, shes had little to no time with her son which broke their connection, that too must be fixed to meet the needs of her son. Ms. Jones, her mother, and the babysitter all must come together to set the same rules and rewards for John.
