ACT HOME COMMUNITY VISIT:
(S): Writer met with the consumer on Friday, 11/3/17 at 1:30pm after the consumer appointment with the doctor the writer went to Social Security Office with the consumer. The consumer needed a copy of his benefits statement for renewal of his low income apartment. The consumer was able to get a letter from Social Security regarding his benefits. The consumer also asks the benefit worker why his benefits was lower. The benefits staff inform the consumer that his benefits was lower because Social Security was paying for his insurance and now that he is receiving Medicaid he can take a copy of the letter from social security and they will pay him for the money they have been taken out for his medical insurance. The writer and the consumer discuss about focusing on his mental health and physical health before trying to get involve with any relationship. The writer inform the consumer that it is hard to be in a relationship when you don’t have your mental and physical health under control. The writer also mention to the consumer that you must love yourself before trying to love someone else. The writer went to the grocery store with the consumer and was encouraging the consumer to buy more than frozen dinner and pot pies. The consumer did buy some ground turkey which he does plan on cooking. The consumer report that he felt like he accomplish a lot today and he is feeling more inpendent today.
