On Tuesday, August 8, 2017, at 9:50 a.m., we arrived near the Insured’s residence at 1200 N Azusa Avenue, Azusa, CA 91702 USA for the scheduled appointment with the Insured, Mr. Henry Rojas. There was one parked vehicle parked in the front driveway at the residence described as a Gray 4-door Toyota CA-license plate number 5PZU752. We approached the house and found the front lawn was manicured with a covered front porch which did not have any visible handicapped ramps, or handrailings were observed. After knocking at the front door, we were met by a male Hispanic subject at the front door who greeted us by shaking the Investigators hand with a firm right grasp. He retrieved his driver’s license from his wallet and allowed for a photograph to be taken of his California driver’s license, and allowed for us to take a picture of him. Rojas with a copy of the vendor letter, while he sat at the kitchen table and completed the Social Security (SSA) authorization.

His identity was verified by viewing his California driver’s license, number C0154968, expiring December 12, 2017, without corrective lenses restrictions. The Insured confirmed his mailing address and physical address as 1200 N. Azusa Avenue, Azusa, CA 91702. He confirmed his date of birth to be October 12, 1962, and stated his full name to be Henry Richard Rojas. He does not go by any other names. He confirmed his social security number of 626-41-9506, and he was born in Jalisco, Mexico. He is a U.S. citizen, and his telephone number is (626) 419-5034.

Prior to, during, and after the field visit, we obtained photographs that depict the Insured’s residence, neighborhood, medications and his California driver’s license. He offered to have a photo taken of the area on the right side below his neck where he showed signs of having surgery. The Insured uses no assistive devices. Therefore there were none to be
