On Tuesday, August 8, 2017, at 9:50 a.m., we arrived near the Insured’s residence at 1200 N Azusa Avenue, Azusa, CA 91702 USA for the scheduled appointment with the Insured, Mr. Henry Rojas. There was one parked vehicle parked in the front driveway at the residence described as a Gray 4-door Toyota CA-license plate number 5PZU752. We approached the house and found the front lawn was manicured with a covered front porch which did not have any visible handicapped ramps, or handrailings were observed. After knocking at the front door, we were met by a male Hispanic subject at the front door who greeted us by shaking the Investigators hand with a firm right grasp. He retrieved his driver’s license from his wallet and allowed for a photograph to be taken of his California driver’s license, and allowed for us to take a picture of him.
