Both of the clients have dementia as their primary diagnosis. One has Alzheimer’s and the other has a mental state… There is also a concern for their comorbidities one has diabetes and the other is on metformin to prevent DM. A nurse would be concerned that the client receives the insulin she needs to prevent any complication that may arise from diabetes. The nurse must also educate the other client about ways to reduce the risks of becoming diabetic. They both are at risk for falls, one uses a walker and one is using a wheelchair. The client who is in the wheelchair is orthostatic so she must be reminded to slow down. A concern for the nurse is when dementia progresses will she understand to take it slowly when standing and if not will there be enough staff available to ensure her safety. As the client with the walker progresses with her Alzheimer will she be able to understand her personal limitations. The nurse must question how the client will maintain her safety if she does not understand her
