On 10/11/2015 SO EMT Perez was dispatched to MG-115 regaurding a fall. SO EMT Perez knocked and announced his presence at the door, and when he received no answer he began to search the apartment. SO EMT Perez found the resident, a Mrs. Patricia Flick on her bedroom floor in a seated position by her bed. Mrs. Patricia Flick slid out of her bed and onto the floor and is unable to get herself up and requested assistance from a security personell. S/O EMT Perez asked Mrs. Patricia Flick if she was hurt or in any more pain usual which she denied. SO EMT Perez performed a pelvic exam also which revealed no pain or grimace from Mrs. Patricia Flick. SO EMT Perez then safely performed a lift assist and helped Mrs. Patricia Flick to her bedside without
