Lewis booked in to see me today due to ongoing concerns over his issues with sleep. He continues to have significant sleep onset insomnia and is still unable to retrial CPAP therapy. He is still reporting some nocturnal wandering, but has not hurt himself or anyone else and does seem to be aware of these events compared to past events where he though they were parasomnias.

Many thanks to Peter Marriot who has increased his Endep to 20mg and clonazepam to 1mg in the last week. He continues to take is Sifrol 750mcg at 8.30pm and is taking 2mg of Circadin at 8.30pm. His restless legs seem to be very well controlled and he will enter the bedroom at acout 10.30pm. This is where his significant anxiety kicks in and he says that he is unable to stay in bed due to the anxiety but not due to any restless legs symptoms. He will become very frustrated and angry about getting to sleep and he still continues to shout with this and is very aware of this behaviour. He believes that he finds it very difficult to control. This will last for up to one or two hours. He then sleeps on and off throughout the night and unfortunately as a consequence of this, finds that he is excessively sleepy throughout the day and sleeps in any passive situation throughout the day.

It appears that this is due to significant re-emergence of anxiety which he ahs recounted is related to his post-traumatic stress disorder. As such, we have discussed some strategies and indeed involving a sleep psychologist again, given that he has seen Professor Gerard Kennedy in the past. He has elected to see Gerard privately and will book in to see him in the next week or

He certainly does have a high pre-test probability of this condition given his snoring, witnessed apnoeas and occasional episodes of nocturnal choking.
