Focus: Skylar’s family will be able to teach Skylar and demonstrate healthy decision making strategies. MS. Smalls (MHP), Skylar, and Mrs. Clark (MHS) discuss Skylar making better choice when completing requests at home and school. MHP, MHS and Skylar review the 90-day progress summary of the Individual Plan of Care (IPOC).
Intervention: MHP and MHS discuss MHS receive information from the school of Skylar not completing assignments. MHP and MHS question Skylar about the receive information and develop a plan to get all assignments completed. MHP, MHS and Skylar review the IPOC progress summary. MHP answer questions about the wording of the goals.
Response: MHS report Skylar teachers called to report Skylar is not completing and submitting
