Reason for Referral:
Shelby is a fifteen-year-old, female that is referred to the examiner by her mother, Natasha G. She has referred Shelby because she needs assistance in the area of mathematics. She is currently taking Advanced AC and DC Circuits for her track in the Engineering program at her school and she still needs help strengthening her Calculus skills. In her Advanced AC and DC Circuits class, she maintains an A letter grade. However, Shelby and her mother would like assistance to continue her development in this area. Her mother states that Shelby is knowledgeable in most aspects of math, but when she has to do homework in this area it takes her much longer and causes her more stress. The area that Shelby needs assistance is considered

Background Information:
Shelby lives with her mother, and her older brother, who's away in college comes back and forth in town during the holidays. Her mother is her primary caregiver and works at the Army Corps of Engineering in her local town. In the morning Shelby wakes herself up for school using her alarm clock, but if she is not up by 5:30 am her mother comes in her room and wakes her up. When she wakes up she first goes to the bathroom to take care of her hygiene. Her hygiene routine consists of washing her face, brushing her teeth, and putting on makeup. After she goes to her room to to get her school uniform, and take it into the laundry room to iron her clothes. After she gets her clothes on she gathers her school books and a breakfast food to go, usually a Poptart, she goes to catch the bus. In the afternoon, she usually has an after school activity to do. On Monday’s, Wednesday’s, and Friday’s she has danceline practice and biweekly she

In her normal environment, her home or with friends, she is very outgoing and friendly, yet when she is in an unfamiliar place she is quiet and reserved. She is known to go out of her way to help friends and family. When her brother was moving out of the house she offered to help him pack and she goes over to her grandmother’s house once a week to help her clean up. Her mother says that Shelby is mostly happy, the only time that she gets upset is if she doesn’t make the grade that she wants. For example, in her history class she made an 83 on a test that she studied all night for and she was sad all day. Since Shelby has so many activities to do in and outside of school her flexibility if high. She enjoys change, for example, her danceline coach told the girls that the would have to practice an additional day prior to the Martin Luther King Jr parade and Shelby was unworried about the addition to her
