The patient is an 80-year-old gentleman who underwent a septoplasty for congenital deviated nasal septum on the day of admission 5/22. Postoperatively the patient was found to be short of breath saturating initially reported to be 92% on room air. He denies any chest pain, jaw pain pain or arm pain, but he had long-standing edema but it was not worsening. His medical history is significant for COPD, diabetes mellitus, hypertension, end-stage renal disease on hemodialysis, small pleural effusion which is chronic, diabetes mellitus, hypertension, stroke and coronary disease having had multiple stents. The admitting physician postoperatively indicates the patient is in acute pulmonary edema. The chest x-ray shows he has a small pleural effusion.
