8/20/2015: As a follow up to the social workers visit on 8/14/2015 to the patient’s home the social worker able to obtain these services through the Senior Center Helping Hands program. The patient will be placed on a waiting list at this time. The social worker will continue to look for more immediate services. The social worker was able to obtain these services through the Senior Center Meals on Wheels program. These services are expected to begin immediately. Social worker contacted safe link and explained the problem with patient’s cell phone and they asked for the patient to call, additionally the patient contacted Safe Link and arranged for a new cell phone to be delivered to his home. The social work was able to locate a used television
