Presenting Problem Jessica is a 28 year-old Hispanic married female client. Jessica presented demanding high stress job and it’s related to her struggle with significant feelings of worthlessness, and shame due to her inability to perform as well as she always had in the past. Client feels unusually fatigued, difficulty to concentrate at work, her coworker noticed that she is irritable and withdrawn, and she stays in the bed all day. Her husband noticed that she has shown little interest in sex and has difficulty falling asleep at night because of insomnia, and she is having a tearful phone conversation over the phone with her friend. She is having thoughts of her does not want to wake up in the morning, and she feels frustrated with herself.

Jessica has loss of interest, markedly diminished interest or pleasure in almost all activities. She feels worthless and diminished ability to think or concentrate. These symptoms caused clinically significant distress in the social area of functioning, also the feeling of loss from losing her clients. So the initial diagnosis will be a Major Depressive Disorder, Moderate with a single episode 296.22 (F32.1).
Jessica has moral and spiritual issues are present which could be her strong faith in God and she has attended the Seventh Day Adventist church, but the point as she reported that she stated that even God feels distant during this time. Also, her feelings of loss and guilt of losing the clients, especially her last client is a moral issue for her due to her obligations of helping and serving others. It may reflect herself image as a high achiever especially because she graduated with top honor in both college and medical school and she has very high standards for herself.
