Presenting Problem: Dylan is a 17 year old with previous psychiatric history of ADHD, cannabis use disorder, severe, major depressive disorder and history of suicidal thoughts with multiple inpatient psychiatric hospitalizations He currently resides with his father. He has a probation officer due to ongoing truancy issues and inability to stop smoking marijuana. He is under advisement due to passion of Marijuana until 03/17/17. He lacks structure and accountability while residing with his father. He report he feels depressed most days and inability to concentrate in school. His most recent admission was 9/2016 due SI. He also admits that binged on cocaine. Given the he has 3 psychiatric hospitalizations, over the past year and has been …show more content…
His parents divorced when he was 7 an he lived with his mother until age 10. Extensive family history substance abuse issues I mother, father and brother. Dylan has two older half-sisters on his mother’s side, one of whom also suffers from addiction issues. Other currently draws disability; apparently form an old head injury suffered a motor vehicle accident while she was under the influence. She was reported an RN but reported lost her license. Father also has history substance use disorder but has reportedly been clean since prior to Dylan’s birth. He has been treated for depression. Per records Dylan mother had a “mental breakdown” after his older brother ran away from home for one year with an adult woman as they were having romantic and sexual relationship. She was hospitalized at this time and apparently “lost ownership” of the family home, prompting a move to W. VA where she resides with family. Dylan and his brother reside with his father. Dylan reported started having behavioral issues during this time period and was referred to a child therapist. He currently in general education, there has not been any concern for his academic abilities. Yet the only issue the school encounter was his ADHD, also him not completing his work due to lack of motivation. He goes to Patrick Henry High School and is in the 9th grade. He states that its overwhelming as there were too many
