D: Client attended last IOP group on 01/10/2017. Last individual was on 02/15/2017. Multiple phone calls were made in an attempt to re-engaged client back into treatment. Client failed to return phone calls. Client was disengaged from treatment and unreachable.
A: Client appears to be in the pre-contemplation stage of change at this time.
P: Client is being discharged from treatment due to lack of engagement for treatment. / HA, CDP
