Client entered Broadway House Women Shelter on 3/31/2017, as a transfer from 85 Lexington Avenue and has been residing in the Shelter System since 2/20/2014. Client is a very intelligent, refined, professional retired journalist. She is very thin and weighs approximately 105 pounds and is approximately 5’10” inches tall. Client is of the olive skin complexion with blond short straight hair. Client report no tattoos or scars. Client was dressed and groomed appropriately

One from The Northern Trust Company Benefit Payment Services in the amount of 4404.01 and the second one is from Pacific Life Insurance Company in the $109. She also receive SSRI in the amount of $1,462.00I. Client has an active Medicaid & Medicare medical insurance. Client also receiving Food Stamps in the amount of $172.00.

Ms. Yafie reported that she was born and raised in New Brunswick New Jersey, and states that she was raised by both parents. Client continues to reports both of her parents are deceased. Client continues to mention that she is the only child; and reports no foster care, domestic violence, military service or criminal record.

Client reports at 17, she left home to go to college; and she continues to report she never being married or have any children. Client mentions she had two cats that she placed in foster
