Introduction
The traditional role of the nurse has significantly changed over time. Some decades ago, nurses were simply assistants or helpers to the doctors and their role was taking and obeying orders from the doctors. Hence, the nurse had no room for making any decision, and as such, the nurses were always viewed as inferior to doctors. However, today the role of the nurses has greatly changed. The IOM 2010 report on the “future of nursing” provides a greater role for nurses in healthcare change. The nurses play a vital role in leading change for the establishment of standard, safe, holistic and patient-centered care (Small & Small, 2011). Campinha-Bacote (2011) describes cultural competency as continued efforts to identify and address the cultural needs of every patient. It is vital for every nurse to be culturally-competent. This is one way of providing patient-centered care. To become culturally competent, the nurse requires to possess the cultural skill. Cultural skill is described as the nurse's ability to collect the cultural data from the patient's condition. Cultural data collection occurs during the cultural assessment phase of care. Tools such as LEARN ("Learn, Explain, Acknowledge, Recommend and negotiate") can be used to conduct a cultural assessment (Campinha-Bacote, 2011). Campinha-Bacote(2011)notes that culture can be a source of conflict between the patient and the healthcare team. For that matter, it is important for the nurse for the nurse to be aware of the cultural needs of the patient and establish measures to achieve them. The fourth and the last week will involve drawing the conclusion from the articles and websites reviewed. This part is very important. It is where the basic knowledge will be acquired for translation into practice. During the analysis of the materials, I will also make useful comparisons with already learned information through discussion with nurse leaders and colleagues. The research literature will provide important evidence-based data for the application. The peer-reviewed article and credible websites only publish data obtained scientifically, making it useful for the application. Hence, the articles and websites will provide important information regarding the description of patient-centered care/individualized nursing care, its core components and as well as the theoretical/conceptual framework involved in its implementation. Moreover, the materials will provide information on the challenges likely to be faced in efforts to implement patient-centered care as well as strategies to overcome them (Debra, 2015). Therefore, by the end of the action plan outlined above I expect to bear the knowledge and skills required for the provision of patient-centered care.
