As the representative of our unit for the Wound Champion Council, I would like to recommend a product as a solution to prevent acquired pressure sores in our unit. If recent NDNQI audit shows, our unit still struggles to prevent or identify our high-risk patients for pressure sores. This audit reveals that our current method of “It takes two” nurses on checking patient’s skin on admission is not reliable and insufficient in preventing pressure ulcers. After my extensive research of looking ways to decrease the rate of unit acquired pressure ulcers in our unit, there is one particular product that stands out; the SEM scanner. I will discuss in details how this scanner is beneficial to our unit.

Introduction: SEM Scanner

The top leading wound care expert in collaboration with researchers from different universities develops SEM scanner. These are highly specialized individuals in wounds, so it is just fitting that they developed this device. Moreover, multiple researchers and different kinds literature show how effective SEM scanner is; this gives the product a trustworthy credential.

Minimal technical skills to operate. This scanner is user-friendly and easy to use. It does not require a lot of skills for nurses or other caregivers to use it. Furthermore, SEM scanner works like a bladder scanner, so nurses will not have a hard time learning how to operate the device.

Non-invasive. This scanner works without the invasive property of other devices. It works just like an ultrasound on the superficial layer of the skin. Likewise, the non-invasive property of this device is favorable to our patients that have a higher incidence of refusing care.

Early detection. The SEM scanner can detect early signs of pressure ulcers on patient’s skin. By using this device on admission with in conjunction with “It takes two”, gives a high probability of capturing pressures ulcers and executing suitable pressure management care. Also, SEM scanner can be used to in any type of skin

On Thursday, August 24, 2017 I got the experience to follow Horizon Specialty Hospital of Henderson’s Wound Care Team. During this experience, I was able to witness the removal of a wound VAC (vacuum assisted closure) and surgical stitches, different methods of wound monitoring and discussed the healing process. For new admissions, the Wound Care Team does a physical examination of the body, looking for any wounds, bruises, scars, tubes and IV placements. They take pictures, measurements and note the location of the wound. Since the hospital still uses paper charting, they handwrite all of their notes and upload them to the patients' medical chart when done with their assessment.…

Bedside LUS should be lightweight and compact for easy transportation and access in the ICU. A paper recorder and a high performance screen should be ready at hand for easy transmission of medical records and a clear view of the lungs. Due to the nature of the ICU wards, a de-infection procedure should be in place with each use as the probe of the ultrasound comes in contact with the skin of many patients and can easily become a vector for transmitting antibiotic-resistant bacteria within the ward. For the lung ultrasound, the convex array probe (3 to 5 MHz) found on multipurpose ultrasound machines is sufficient for the visualization of the lungs. The probe should be placed perpendicular to the chest for each intercostals space for…

I think the problem this research is trying to address in respects of Medicare reimbursement. Medicare, in most cases, will decline reimbursement any pressure ulcers unless they are present on admission (Wilkinson & Treas, 2011). The key for this matter is the accuracy and completeness of the documentation upon patient’s admission assessment. The true research question posed in this study I read about is “which one better in terms of accuracy and completeness of pressure ulcer documentation? paper charting or electronic…

The transducer is then guided across the patient at different angles to render different views of the heart and vessels. The sonographer will sometimes have the patient lay on their side with an arm raised for imaging. Another view requires the patient to lay supine with a pillow directly under the neck so the head hangs and the transducer is placed at the suprasternal…

A Primary Source Critique Paper The research problem that I have chosen in Unit 1 was pressure ulcers. This problem needs to be researched in order to properly address such problems in nursing home settings and to be able to find ways to at least prevent if not solve the problem is not only promoting positive patient care outcomes, but also increasing the quality of care of the nursing home facilities. As nurses, we are encouraged to apply evidence-based practice in our profession. In finding evidence we have to determine the validity of the source.…

Nurses during the 1970s and 1980s used to perform “back rounds” in order to ensure that patients’ positions were being changed every two hours in order to prevent the formation of pressure ulcers. This practice is still implemented today. In addition, hourly rounds now consist of nurses checking on their patients’ general well being and the 4 P’s. The 4 P’s include positioning, personal needs, pain, and placement (Forde-Johnston, 2014,p.38). The 4 P’s helps nurses, nursing assistants and clinical supervisors perform rounds in a more organized manner, providing a checklist to help ensure patient safety and comfort.…

When patients are in the hospital, they are often tested for many diseases and conditions. In fact, many hospitals have clinical testing areas and labs for testing saliva blood, and urine. However, these labs require a large amount of manpower and time to test samples, collect data, and analyze results. POCT (Point-of-Care Testing) is bedside testing for patients. Although POCT technologies provide many benefits such as, reduced testing time, decreased turnaround time, increased portability, and improved accuracy of results, high costs have inhibited many facilities from purchasing and using POCT technologies.…
