About the dates in question about the claimant’s date of injuries of August 5, 2017, as well as July 26, 2017. Mr. Raigoza claimed he was working on both of those times as the only information that he is aware of involving slip and fall of a customer was reported to him on Sunday, August 6, 2017, at approximately 5:00 PM. He claimed that he did not have the schedule for who was working on both of those dates in question and did not have the authority to ascertain that information to provide for this investigation. He said any information that is required to find out who was working on as well as obtaining any security video footage within the mall is to be done through writing to the Director of Security, Mr. Daniel Luna. He did not have Mr. Luna’s contact information to provide for this investigation.…
On Thursday 03/29/2018 at approximately 2205 I was instructed by Lt. E. Gonzalez #122 to assist Officer E. Hernandez #216 at Marina Hall (located at 6515 Ocean Drive Corpus Christi which is on Texas A&M University-Corpus Christ) with talking with the neighborhoods. Upon arrival I activated body camera #5 and made contact with an individual by the name of Austin Lucero (student/resident) who lives next door in apartment #2121. I advised Lucero about the incident that occurred next door. I asked Lucero if he has been in his apartment all day, which Lucero advised that he arrived to his apartment a little before 1920 and was at the lobby area of the building until 1930 when he headed to the food truck and did not come back until approximately 2120 to the apartment.…
On 10/15/15 at about 0930 hours, Sgt. Aksdal, Detective Davidson and I were working in the investigations office at the Kirkland Police Department, 11750, NE 118th St. Kirkland WA. Emergency…
Mr. Jacobo has been employed with the insured, for approximately seven months as a Line Assembly Technician 1; the same position which the claimant, Mr. Donald Arauz held when he was employed with the company. His job typically requires him to sit down in front of a worktable and to assemble by small hand elements of screws and other hardware in an assembly base motherboard or items that require the equipment to be either screwed-in with a small hand-held screwdriver or with an adhesive glue called HYSOL. They also use other smaller portable tools such as tweezers, hand-held air pressure syringe, tongs while they would hold these items in either hand. By doing so, they would use their fingers occasionally for 7.5 hours to grasp these tools…
9… Candidate Allison lacked command presence when she issued her order to her fire team. SNC was visibly uncomfortable with the five paragraph order and stumbled through the order. SNC did not brief the Admin and Logistics paragraph nor the Command and Signal paragraph. SNC made references that alluded to sections within the five paragraph order, but there was no connection as to the five paragraph order itself.…
SNC took four minutes of his allotted ten minutes developing his brief; SNC’s fire team did not step off to execute until after ten minutes had elapsed. SNC’s Situation paragraph contained detailed information about higher and adjacent units that was not briefed to him. SNC briefed a scheme of maneuver that was based off tactical control measures that he was unable to explain after he was asked what they meant. SNC’s tasking statements did nothing other than assign fire team billets to subordinates; SNC proceeded to brief later on that tactical control measures were not applicable. SNC briefed that the squad leader was with his fire team, which was not accurate.…
SNC briefed OSMEAC completely with confidence and competence. However SNC did not brief or implement security personnel or procedures during the course of the event. SNC maintained strict control of ninety percent of the events movements which demonstrated a good working knowledge of what needed to happen but lacked confidence in his plan and showed a disbelief of his fire teams capabilities. During extreme points of control SNC micro managed the smallest movements of each candidate without taking the individual candidate’s capabilities into consideration. This led to a point of friction between the fire team that caused a delay in the movement forward and a breakdown in communication.…
SNC established security prior to brief. SNC was unable to provide the accurate Azimuth in the allotted time. SNC provided a disjointed and incomplete order. SNC started with a vague Orientation. SNC confused the fire team leaders when he blended the information together and failing to state, "Concept of Operations, Scheme of Maneuver, and Tasks".…
On 13 March 2017, PV2 Santiago you were not present for the mandatory squadron accountability formation held at 0850 at the squadron motor-pool. You also failed to notify me or your chain of command that you would not be present. I received a text message at 0827 from PV2 Johnson that he dropped you off at Bennett Clinic after PT formation because you were throwing up. At 0915, I contacted the clinic and was told you were not there or was seen that morning. I then to Darnell ER, EBH (Embedded Behavioral Health)…
Inside the law usage industry certainty and motivation is two primary edges to have remembered the actual objective to meet a complete target of accomplishment of any division. This logical examination delineates how Captain Edith Strong could make sense of what she needs to do to bring the certainty and motivation up within the road watch unit for the workplace. According to More, Vito, and Walsh (2009), the officers were feeling in the going with courses about their occupation: "act as frustrating, calm affirmation, long response times, and grievances by subjects, inadequacy cases, and quick specialist turnovers. " I will make sense of what I would do if I were Captain Strong to raise the soul and get all officers motivated and if E.R.G. would…
As seen on the listed shift assignments schedule you have sent out, Officer Nathan has been moved from Shift #2 to Shift #1, and then when the shift rotates, he again is changed. There appears to be some scheduling favoritism within the shifts. Shift #2 has been short staffed for over two years. You have verbally agreed that this was the history and you were working on correcting the problem. I had asked Sgt Kisner if Ofc Nathan had been reassigned; to which he stated “Not that I have heard”.…
In September 2013 Jessica Bennett, Rusty Shingleton & Matt Johnson had me come into Jessica office to explain that SOT (Select Oilfield Trucking) would roll up into SFS (Select Field Services) and at the time SOT had Christa Wigal and Mary Williams. Christa Wigal was the processor and Mary ran the scales. Christa Wigal had moved up to the now Select yard prior to the roll over to SFS due to getting rid of the office trailer at the stone yard, to save money monthly. I was told during my meeting that I would be the supervisor over Christa and Mary but was not to say anything yet but to start streamlining processes for SOT to roll into SFS.…
Good afternoon, On 6 May 2016 I had a brief discussion with Stephanie Ross of Internal control regarding security posture for the upcoming dismissal of employees that conduct business within the Charles F. Hurley building. We agreed the building needs to be staffed with additional security during this time to safely conduct daily operations and to allow for flexibility in the event of multiple incidents and to provide a security presence in the form of interior patrols.…
On Sunday, September 27, 2015 at 2330 hours while I was sitting stationairy on the unit block of W. Centre St. near the Lyric the COMPLAINANT walked to my patrol car and advised me there is a physical fight in progress in front of the Mexican Bar. I advised the COMPLAINANT I am going there now and told the COMPLAINANT to walk back over to the Mexican Bar so I can get his information. I arrived at the Mexican Bar located at 8. E. Centre St. and activated the emergency lights on my patrol vehicle. I observed two white males yelling at a Hispanic man.…
It was October 8, 2015, I walked into the police station not knowing what to expect. When I walked to the desk they asked me for my name, and Patrol Officer Jonathan Bransfield greeted me. I was brought behind the desk into the dispatch center where I filled out the paperwork to go on the ride along. After signing the paperwork Officer Bransfield showed me around the station where I was greeted by everyone. Officer Bransfield showed me the temporary holding cells, the booking room, the chief’s office, the severe-weather reporting room, and the detective’s office.…
