The patient is a 39 year old male who presented to the ED with substance abuse concerns. The patient denies suicidal ideations, homicidal ideations, and symptoms of psychosis.
During the time of the assessment the patient calm and 4X oriented. The patient reports this past December he wife died. The patient reports having kidney stone and being prescribe Roxicodone. The patient reports that after being prescribed this medication he relapsed. Th patient reports using Opana earlier this morning. Th patient reports withdrawal symptoms of Nausea/ Vomiting, cold sweat, feeling irritable. The patient feelings of depression. He reports, isolating himself, and insomnia. According to the patient most recent drug screen he tested positive for Oxycondone.
