We are in the process of conducting a routine audit of provider claims. In connection with these audits, we are requesting the complete medical records for the patients listed in Attachment A and attached hereto. Please include any and all documentation for the care of the patient related to the dates of services, including but not limited to: • Complete Progress Notes
• Treatment Plan
• Assessments/Testing
• Medications and/or Labs

Copies of these records must be received within 14 business days along with a signed copy of Attachment A. A signed copy of Attachment A must be included with the records or the documentation will not be considered in the audit. Any missing medical records/documentation may adversely affect the results of this
