1) A maintenance order was submitted to the Roudenbush Facilities Director to have the shelving unit properly secured in the yellow classroom. The facilities team has successfully resolved this issue.
2) A maintenance order was submitted to the Roudenbush Facilities Director to repair and reduce the gap in the side gate. When the fence is more accessible, it will be repaired. Children will no longer have access to the playground or fence until it is repaired.

3) The stairwell where the radiator is located is no longer being used as an entrance/exit point for the school-age program. Children and staff now follow a safer route by using the front doors of the Roudenbush Community Center.

-Staff members missing physicals & mmr’s have been updated and placed in files.
- Staff has been retrained on Look before You Lock and certification of training completion has been placed in all staff files.

19) Child’s file has been updated with the appropriate forms to indicated evidence of current first aid/emergency medical consent release, a current off site consent, and a current transportation plan. All other student files have been reviewed and updated where needed, as well.
20) Current program director is continuing to complete monthly staff meetings and providing evidence of the completion of these meetings. There will be no lapse in meetings under the current director’s administration.
21) Supervision and observations have been and will continue to be done on a monthly and/or bi-monthly basis under the direction of the current director. Evidence of the observations/supervision will be placed into staff member’s files.
22) Current program director is continuing to complete evacuation drills and record them in evacuation log in order to provide evidence of completion of the drills. There will be no lapse in meetings under the current director’s
