On 9/19/16 I met Ms. Ostrander at the office of Dr. Hettle PMR. Ms. Ostrander reports she still has pain in the left shoulder, mid back and the low back. Her cervical pain was from a prior cervical surgery. She said that she saw Dr. Schell on 7/12/16 and was coming home from that appointment when she was his in her car. Dr. Schell told her at that appointment her prior cervical disc repair had not healed and that he would need to go back in to re-do her surgery. Ms. Ostrander said that the Mobic twice daily and the Flexeril at night helps. Dr. Hettle said the MRI of the Brain was normal, and the MRI of the left shoulder did not show any tear to the Rotator cuff. It did show a tendinitis to the shoulder. An EMG was done of the upper
