The coach will be responsible for scheduling a home visit within 72 hours of discharge and Post-discharge. Ensuring that our obligations to keep patients safe and reduce their chances of readmission are successful. Another important Part of our nursing team should be a care coordinator. This nurses’ duty would be to keep a record of all high-risk patients who have the possibility to be readmitted. The coordinator would contact the family and patient after discharge ensuring that the patients have received the care they need and that they understand their medical condition, medications, and other instructions.
