It is obvious that she did not take responsibility for the documentation. Along with this, nursing handover should be conducted appropriately and ineffectively. Communication is one of the main risk factors for adverse events in clinical settings. Effective clinical handover significantly reduces communication errors and ensure a safe and a high quality of health care delivery (Bruton et al. 2016, p. 386). At the same time, through handover, nurses transfer their responsibility and accountability of care to the following nursing staff (ACSQHC 2012, p. 7). There was also apparently inappropriate communication that the nurse did not perform a handover accurately. Accurate handover also relates to ethical aspects that nurses have an ethical responsibility for detecting and preventing errors and adverse events in an early stage by sharing relevant information for patient care (NMBA 2008a, p. 5). Ms Colleen Price conveyed her subjective comments and did not inform any relevant information, such as any changes in Mr. Lee, medication and nursing evaluation. If the nurse informed such accurate information, the morning staff could detect that problem although the re-operation could be already unavoidable at that time. Therefore, in order to prevent the clinical incident, concisely and accurately communicating with colleagues was obviously an ethical issue for the nurse and she did not performed this communication, which can be a contributing factor for the clinical
