Richard Bellamy was referred to Adult Protective Services by Wellness Coach Ms. Cain due to him using substance of crack cocaine and k-2. Mr. Bellamy has a history of leaving his residences for more than 30 days and sleeping outside between 34th street Penn Station and 42th street port authority.
On 4/10/2017 APS Case Worker Waveney Atkins came to do an Assessment for Mr. Bellamy. Per conversation with Ms. Atkins, Mr. Bellamy did not let him in his residence. Mr. Bellamy came downstairs and appeared alert and groomed. Mr. Bellamy greeted staff, Ms. Atkins express that she is case workers for Adult Protective Services in which will assist Mr. Bellamy with his financial management. Ms. Atkins asked if Mr. Bellamy would like to accept services and he replied “yes ma’am”. Ms. Atkins asked when the last time he used was and he replied the first week of April 2017 and the drug of choice was crack cocaine.

On 4/12/2017 Wellness Coach Ms. Cain went to Mr. Bellamy apartment to see if he has attend his appointments. Wellness Coach Ms. Cain knocked on the door and Mr. Bellamy appeared alert and groomed. Mr. Bellamy express that he did not attend his appointments and is going to walk in to the Manhattan VA on 4/13/2017. Wellness Coach Ms. Cain counseled Mr. Bellamy on the importance of his health as well as mental health. Mr. Bellamy replied “yes ma’am”.
Later in the day Wellness Coach Ms. Cain called Ms. Atkins to follow up on Mr. Bellamy rental arrears. Ms. Atkins informed Wellness Coach Ms. Cain that as soon as the check gets printed it will be mailed to HELP USA. Wellness Coach Ms. Cain thanked Ms. Atkins for the following information.
Plan of Action: Wellness Coach Ms. Cain will follow up with Mr. Bellamy on his overall health. Stage of Change:
