. Down’s syndrome is a chromosomal disorder that involves both a certain amount of physical deformity and some degree of mental retardation. 49 Esophageal atresia with tracheoesophageal fistula means that the esophageal passage from the mouth to the stomach ends in a pouch, with an abnormal connection between the trachea and the esophagus. As a result, food and drink pass to the lungs instead of the stomach, eventually resulting in suffocation unless surgery is performed to correct the malformation.50 Surgery to correct esophageal atresia with tracheoesophageal fistula is routinely performed with success, but the parents of Baby Doe refused to consent to the surgery. A potential ethical dilemma that I could face in my chosen advanced nurse practice role as a family nurse practitioner is a large family that disagrees amongst themselves over whether to continue their mothers care. The situation is this: A mother of five children, divorced for over fifteen years, enters the hospital after suffering a massive stroke, was found unresponsive yet breathing in her yard by a neighbor. EMS was called and she was brought into the hospital and placed on advanced life support. The primary ICU physician and neurosurgeon are in agreement that the patient will not regain a significant amount of brain function. It has been a catastrophic injury without hope of recovery.…

This building also includes a therapy unit for people who have had a recent surgery and need physical therapy during their recovery process, a unit for retired people who can no longer care for themselves at home alone, and a unit dedicated to people with a tracheostomy that require a ventilator to assist them in breathing. A tracheostomy is a surgical opening through the trachea. This is necessary for someone who is unable to maintain their own airway. The ventilator is connected to the tracheostomy which will then maintain a constant airflow for the patient to breathe. For the first time in my three years of employment at this facility I worked on the “vent” unit, as it is refer to it in the medical field.…

I believe that the most important teaching/learning aspect that the caregivers, both medical and family, needed to understand when caring for patient #17 was the concept of AND. To put it into perspective, it isn’t unusual for the elderly population to put DNR on their advanced directive – they are elderly and no longer want to be in pain/suffering. However, it is unusual to see a pediatric patient with an AND order. AND, meaning to allow natural death is something that individuals with chronic illnesses will put in their advanced directives. This particular patient requested that no chest compressions or cardiac medications be utilized in the case of distress.…

Ethics Ignored for Henrietta Lacks In the book, The Immortal Life of Henrietta Lacks, Henrietta was the victim of abuse from the health care delivery system. Her story is remarkable, but as a social worker reading her story there were many read flags involving unethical care to the client. The most disturbing part of the book was reading the consent provided on page 31 of chapter three. When Henrietta presented to John Hopkins Hospital to be treated for her cervical cancer she was given this consent: “I herby give consent to the staff of The John Hopkins Hospital to perform any operative procedures and under any anesthetic either local or general that they may deem necessary in the proper surgical care and treatment of: ____” (Skloot, 2010).…
