Staff and client met for her 6th individual session. The purpose of this meeting was to enhance the therapeutic relationship and to work on her service plan goals. Specifically, we collaborated on Problem #2 Relapse Prevention action step #4, where she is to verbalize if and when she was previously sober what has worked for her in the past and what did not. Revealing that she stayed sober for her daughter in the past, she verbalized that she now takes care of herself first, so she can be a better mother to her daughter. However, when she became bored she met up with old friends and sobriety became less of a priority, which inevitably led her to relapse.
