Rehabilitation Unit Transition Process

In my previous work at the rehabilitation unit, we always have problems with admission process or transition process from hospital to rehab unit, such as administration, medication reconciliation, confirmation from a house physician, lack of staff and coordination. According to AHRQ (2016, para 1), “A classic study found that nearly 20% of patients experience adverse events within 3 weeks of discharge nearly three-quarters of which could have been prevented or ameliorated”.
During the transition process in rehab unit, the nurse needs to reconcile all the medications, approved by supervisor, and then ask the physician for signature and approval. In addition, the nurse needs to make a medication list to the pharmacy. While waiting for this process,

The Rosa Parks Wellness Institute for Senior Health (RP-WISH) created a program that focused on improving the safety of care by making an increased effort to schedule follow-up appointments and medication reconciliation within 1 week of discharge.
