Reflection Report Wound

On 8/9/17 I was contacted by Chesaning Rehab. Ms. Perez had an issue with her abdominal wound. There was continued bleeding after a wound vacuum change. She also had her blood drop so she was sent to the hospital. I contacted the Henry Ford clinic for Dr. Blyden and canceled the appointment.

Ms. Perez was discharged back to Chesaning rehab on 8/14/17. On 8/18/17 I met with her team and Ms. Perez to discuss the current status and rehab plan. Ms. Perez reports she has a lot of pain to her lower extremities from the cellulitis. She was tearful on and off during the meeting with staff. She is down and discouraged. She currently works with speech, physical and occupational therapy. She has a lot of nausea from some of the antibiotics and there is the talk of putting in a PICC line and doing her antibiotics through that. Perez at the office of Dr. Wolohan. This was an extremely long appointment. We were seen by the PA and then Dr. Wolohan. X-rays were taken then mid appointment more x-rays were taken. Ms. Perez was a 4 person max assist to get her to the x-ray table to have x-rays done to the right left ankles and left leg, along with the right wrist. Ms. Perez has severe pain to her ankles and leg and the wrist. We had a lengthy discussion. Dr. Wolohan said she needs to have more surgery on the right wrist, she was given a brace today. He said the left femur is still healing. Her ankles and limited flexion so she cannot flex them enough to stand. He said at some point she may have a fusion of the ankles. He said in the meantime she needs to heal and get the cellulitis under control. She has edema to her legs and her arms. She still has a wound vacuum on the abdomen but that wound is almost healed. She has a PICC line now for antibiotic infusions. She is allowed to weight bear as tolerated and also may work with physical therapy as
