Client is a 32 year Caucasian male with multiple attempts of suicide. The client was transferred from a previous medical institution (Park Wood). The client has a hard time with rejection. The client shot himself after a breakup with the mother of his children. The client had a car wreck which caused him to suffer depression, schizophrenia behaviors, and drug use. The client decided to check himself into a medical institution, but the institution refused to take him in because he did not have a doctor’s referral. The client went into the bathroom of the facility and cut his wrist then the facility admitted him, then the facility released him to the North Mississippi State Hospital. The client is around 5’8; very thin build around 130lbs.
II.
Background and Current Functioning The client was raised with his mother and father, he has one brother and he is the youngest. Both parents are alive, but they are separated. The client’s main support is his grandmother; he is his other providing parent. The client suffers greatly since birth because his mother was electrocuted when she was pregnant and that is how he ended up with birth defects. The client had multiply surgeries on his hands, so he can use his others fingers for thumbs, and the surgeries were successful he can use his hands well. The client’s father uses crack and cocaine, his brother uses heroin and his aunt suffers from bipolar and schizophrenia disorders. His mother works at a casino and presently lives with his grandmother. The grandmother stated, the client can live with her until he get himself together and she will pay his bills, so he can return to his own home. I will speak with family to work on discharge planning. The client will stay with North Mississippi State hospital 7-10 days for further evaluations and the treatment will decide his medication for treatment and his length of stay. The patient will be given a packet or a folder with all the necessary information about his rights and hospital policies. The client will also be given a Seclusion/ Restraint Form, which gives the client the right to have someone called if he becomes a threat to himself or a threat to others. The client’s goals are to “keep his medication on track this time”. He plans to go home to take care of his children, his grandmother and hope to find treatment for his older brother’s addiction. The client wants outpatient help for himself even though he says he is not an addict. The intervention plan is to have him in an evaluation by social workers, psychologist, counselors, nurses and the doctor to see how reacts to all forms of treatment methods to make an adequate diagnosis. At this time the only barriers is his Dyslexia and his ability to get along with others at the facility. The patient is required to participate at all didactic groups, goal settings, recreations, and cooperate with taking his medications. The client has been given a cigarette patch in place of a cigarette, which he sometimes declines because he is trying to quit
