My patient for this week was a young man that admitted to hospital due to pancreatitis. He is independent on all the daily living activities. The only thing I needed to care for him is to make sure his pain level is under control. After I conducted my head to toe assessment, I went to prepare his medications. Since his pain level is at 10/10, the nurse has to give him his pain medication through IV around the clock. After my assessment, the patient requested me to allow him to have rest time because he is really tired. So I respected my patient’s request by leaving him alone and only went to check up on him when the pain medication is due. During the time that my patient was resting, I went around the unit to answer patient’s call light. Two patients besides my own patient that I help during this week’s clinical make a difference in my clinical experience. One patient was one of the patient that I answered the call light for; he is a very young man that just finished his surgery on

Clinical Context & Experience I completed my final two shifts on 11/24 and 12/1 at Sharp. The unit had a pretty large census both nights, so we were fairly busy both nights. During this shift I had to notify the doctor twice and felt the most relevant journal topic was advocacy. The first night I had three patients which included a 84 year old man who sustained a hip and right shoulder/humerous fracture from a fall at home and a history of HTN, CKD, MI with coronary stent placement, and diabetes, an 86 year old woman admitted for rapid A-fib with history of TIA, HTN, CHF, and rheumatoid arthritis, and finally a 35 year old woman who was being discharged following her treatment for polynephritis. During my final shift, I cared for a 71 year…

During my clinical rotation at St. Joseph’s Regional Medical Center, Patterson, I had the opportunity to be with the patient at the time of physician’s rounds. The physician questioned the patient and me to know whether either of us had any concerns about the medical care being given. I was able to advocate for the patient and bring up some of the patient’s concerns. I was also able to reinforce the instructions given by the physician to the patient after the visit was completed. The patient seemed really happy and satisfied about the care he had received.…

I got back to the first patient’s room after about 4 hours. When I entered her room, she said she was in pain and asked for another morphine before I do the dressing change. I was not ordered to administer another morphine at that time and the order that I had for that patient called for morphine to be administered every 8 hours. I knew that no matter what I had to change her dressing and at the same time I did not want her to be in pain. I tried to explain to the patient why I was not just able to give her the morphine as she requested but she got mad and threatened to report me.…

This means that I will have to do thorough assessments all every patient, no matter what they are in for. When Teresa was interviewing a patient for the first time, she remained very focused on what she was doing even though the patient insisted on sleeping. She had to get him to drink a powdered potassium even though he tried to put it down after each sip. Another patient, did not want any more antibiotics because her IV site was very painful. Teresa told her that after she took one more, she would remove the one that was causing pain.…

Collaborating with the client is very important in the nurse – patient relationship as we need to work hand in hand to achieve the planned goals. I had this patient who had a Congestive Heart Failure, who was experiencing difficulty of breathing, with productive cough and was restless. As a nurse, following the nursing process, collaborating with the client at this point was addressing his complaints, attending his needs and concerns and for me to do that, I did an assessment for both subjective and objective data. Also, I gave my priority nursing intervention which was to position the client on bed comfortably in a fowler’s position and ensured that he was receiving an adequate amount of prescribed humidified oxygen. Afterwards, I informed my Clinical Instructor about it and of course the staff nurse on duty and verified my assessment.…

My personal experience and take away from English 111 was what I expected it would be before I started the course. No, there was no “life changing” moment for me from this course and I honestly I can’t say I am a better or worse citizen for taking this class, but I can say my writing skills have improved, and that I feel more confident in my writing than prior to taking this course. Writing has and continues to be a challenge for me, I have no illusion that I will ever be a “perfect” or even a “good” writer for that matter, but because some of things I learned in how to prepare and to write an essay I feel I’m a much improved writer. Specifically the topics covered from the first module of this course help my development.…

My experience in the emergency department at Legacy Salmon Creek during my paramedic education gave me insight to the patients I hope to eventually serve. During this experience, I got the opportunity to help patients suffering from life-threatening illnesses. However, the majority of the patients I worked with did not have emergent conditions. I realized that many patients visited the emergency department because they did not have access to affordable health insurance for preventative care. Emergency visits are expensive and the wait time could be detrimental to patients.…
