Reflection On Completed Dca Health Care

Decent Essays
Completed ACA health care. Assisted with case management by answering phones making appointments and shadowing Task manager. I went to the DCM hospital; first time shodowing Andrew the care coordinator. 2nd time interview client without Andrew assistance and completed MI-Bridges application with client.

hadowed (EIS) Denise in Triage with an client that was blind and death living with HIV. We assisted him in filling out applications for housing. I continued working with TM Kristy in case management this week and attended Weekly supervision meetings with supervisor Jennifer Shwarts every Tuesday; discussions of weekly tasks.
