Per Court Order signed by the Honorable Schulman (4/27/16) and at the request of the Agency for Persons with Disabilities, Nathaniel was referred for an evaluation to determine whether or not he meets the diagnostic criteria for intellectual disability or autism. Nathaniel currently has one open case which includes a Burglary Conveyance unarmed for an incident that allegedly occurred in January of 2016. The youth is scheduled for a Court appearance on this matter in the near future and the present assessment was ordered to assist with that process.
CONFIDENTIALITY STATEMENT:
Nathaniel was not in custody at the time of the assessment. Thus, he was evaluated in this examiner 's private office located in Coral Springs, Florida. Nathaniel communicated within the English language, and was informed of the nature and purpose of the current evaluation. He was notified all information obtained during the course of the evaluation was not confidential and any reports of abuse, or intent to harm self or others, would be reported to the appropriate agencies. Nathaniel was informed that subsequent to the evaluation this examiner would provide the Agency for Persons with Disabilities and the Court with a report summarizing all relevant information and key findings to assist with treatment planning. He was told speaking with this examiner was not required, and he could decline to answer any of the questions posed by, or presented in the assessment measures.
