Andrea is a 27 year old single unemployed female referred by Elmhurst Hospital due to a suicide attempt by overdosing on Zoloft. Client has ongoing mental health symptoms which impair her ability to function and adequately care for her 9 year old son who has been diagnosed with autism, and is currently in foster care. Client has a history of mental illness, has had frequent hospitalizations, received prior mental health treatment. Client presents with depression, feelings of sadness, difficulty communicating and difficulty maintaining positive relationships. A summary of current sources of Evidence- Based Assessment for Individuals include back ground information on clients from case records, verbal reports from clients, direct observation of nonverbal behavior, observatioin of intearaction with family members and others, collateral information from families, relatives, physicians, teachers, employers, and other professionals, and tests or other instruments(Holosko, Dulmus, & Sowers, 2013, p.132-133). Conducting a through Evidence- Based Assessment this writer would educate the client on the informed consent and mandated reporting practices, listen empathetically while expressing genuine concern with presenting issues. Writer would inform the client that the primary purpose is to assist the client with obtaining her treatment goals. Writer would provide full attention to client while obtaining information.
