Physical Examination/Objective Data Eyes: When central vision tested (removed reading glasses) using the Snelling eye chart, no hesitancy, squinting, leaning forward or misreading letters. Visual acuity in bilateral eyes 20/20. Near vision was tested with reading glasses on, with result 14/14 in each eye, without hesitancy or moving the newspaper further away. Results of the peripheral vision or the visual field test included, finger was seen (by both of us) at about 60 degree nasally, about 50 degrees superiorly, about 90 degrees temporally, and about 70 degrees inferiorly. When corneal light reflex tested, reflection of the light on the corneas was exactly the same spot on each eye, and white dots seen symmetrically. During the cover test, uncovered eye remained with steady fixed gaze. No muscle weakness observed, as the uncovered eye stared straight ahead. During the diagnostic position test, leading the eyes through the six cardinal position of gaze, none of eyes produced any muscle weakness during the movement. No nystagmus, no lid lag observed. Further, on inspection of the external structures, it was observed relaxed facial expression, …show more content…
The skin is pink-red, consistent with the client’s facial skin color. Further, skin is intact, without the lumps or lesions. No evidence of tenderness when the pinna moved and when pushed on tragus. No evidence of pain observed, when palpated mastoid process, bilaterally. The external auditory meatus without swelling, redness, or discharge. No cerumen present. The examination by the otoscope showed no redness and swelling, lesions, foreign bodies, or discharge in the external canal. The eardrum is flat, slightly pulled in. It is shiny and translucent with a pearl gray color, and is intact. The client correctly repeated the whispered numbers, “999,” and “666,” when his hearing was tested bilaterally. Sound heard equally in both ears with the Weber test. The Rinne test showed that an air-conduction is twice as long as the bone conduction.
Interpretation of Data:
Eyes: Healthy vision function and healthy eye structures: visual acuity 20/20 bilaterally, sclera white, conjunctiva pink; pupils constrict 4mm to 2mm; equally, round, and reactive to light and accommodation (PERRLA); disk margins sharp, no hemorrhages or exudates.
Ears: Healthy ear structures and accurate hearing: acuity good to whispered voice; tympanic membranes (TMs) with good cone of light; Weber: midline; Rinne: AC>

Test eight and ten evaluates the function of cranial nerve eight, the vestibulocochlear nerve, which is important for hearing and equilibrium (balance). Test eight has the patient walk across the room; while she was walking, she complained of dizziness and exhibited balance difficulties. Ordinarily, one would not experience dizziness or inability to balance. When the Weber test is performed, one should be able to hear in both ears, but the patient could only hear in the left ear. Due to the inability to identify different tastes and presented with an asymmetrical face, which suggests that the seventh cranial nerve has been damage, and dizziness along with trouble balancing and unequal hearing, which indicates damage to cranial nerve ten, thus the original hypothesis is correct.…

assess whether the patient hears better by air or bone conduction. For this test, tap the tuning fork on the hand to activate it, and then place the base of the tuning fork on the patient’s mastoid bone. If the vibration is conducted through the bone, the patient hears a humming sound, time this interval of bone conduction, noting the number of seconds when the sound is no longer heard (as reported by the patient) move the still vibrating folk near the auricle ear canal (1-2cm) and ask the patient to tell you when the sound is no longer heard while continuing timing the seconds. Compare the number to seconds sound is heard by bine conduction versus air conduction, normally air conduction should be heard twice as long as bone conduction.…

Preliminary analyses revealed no significant differences between left- and right-averted gaze targets across eccentricities, and no significant effects of participant gender or stimulus gender on gaze discrimination were found. Mean RT data were computed using only correct responses. For each subject, RTs that were below 150 ms (0.1% of the total data) or exceeded 2.5 standard deviations (9.3% of the total data) from the mean of each gaze condition per eccentricity were discarded. All trials in which more than one fixation was made (i.e., when participants moved their eyes away from the centred fixation) were eliminated (2.9%). RTs exceeding 1000 ms were recorded as a miss (7.2%).…

The aim of the experiment was to investigate the change in visual acuity across the nasal retina to an eccentricity of 45 degrees. Method Firstly, the subject was sat down and the Visual acuity was measured using the log mar chart, the decimal V was calculated. A display board was positioned in front of the subject at a distance of one metre ensuring the subject’s eyes were parallel to the board and the Maltese cross was positioned on the extreme left end. This is to ensure the cross was projected on to the nasal retina when the right eye was tested. The left eye was occluded using the occluder in the trial frame.…

If the patient is not on any vestibulotoxic drugs such as anticonvulsants, furosemide, ethacrynic acid, anti-inflammatories, or inflammatories, vestibular disorder resulting from toxicity can be ruled out. Neurologic testing should be done to rule out neurological findings that could produce vertigo such as cerebellar disorders, multiple sclerosis, and vertebrobasilar disease (Towler,1984). If the person does not have tinnitus, or hearing loss, both cerebellopontine angle tumor and acoustic neuroma can be ruled out (Towler,1984). In addition to tinnitus and hearing loss, if the patient does not complain of a sensation of pressure in the ears, Meniere’s disease can be ruled out.…

The structure of the eye contains several complex structures that make the eye like so of a camera. Without using complex equipment to view all parts of the eye or dissecting it, one can see three structures of the eye: the sclera, the iris, and the pupil. The white part of the eye is known as the sclera which is the outer most part, that helps the eye maintain its shape and protects the eye from injury. The black dot one can see in the middle of the iris is known as the pupil. The pupil is in charge of letting light into the eye.…
