Mr. Green receives $290 in cash assistance and $317.00 in food stamps monthly. CM requested an updated budget letter. Housing:
Mr. Green must continue to meet with the housing specialist once a month until he secures permeant housing.

Education:
Mr. Green will ensure his son attends school on time & a daily basis. Employment:
Mr. Green is not employed at this time, however he has an interview with the New York City Park Department May 10, 2018 at 9am.

Medical/ Follow Up:
Mr. Green will ensure that all medical are up to date. Client will inform CM & staff is medical attention is needed. Mr. Green informed his CM he is doing much better. Client was taken to Brookedale Hospital on May 3, 2018 due to chest pains. Client
