This evaluator was contacted by the Department of Social and Health Services (DSHS) to conduct a psychosexual deviancy evaluation and current risk assessment for future acts of sexual misconduct for Gage Martin a fifteen year old male residing with his adoptive parents, Carlie and Matt Martin. This evaluator was informed that Gage will be placed in a residential treatment facility in Spokane Washington and this evaluation will be used to determine if Gage will be in need special accommodations.
Denise Colucci is the DSHS caseworker currently assigned to this case and she states that in the last two months Gage has been in and out of the emergency room for suicidal and homicidal ideations. Gage continues to struggle with boundaries with peers on his hall continuously being marked for being in peers’ bedrooms. Gage constantly asks for a roommate or to be removed from no interactions with peers that he has poor boundaries with. Gage has difficulty understanding what is and is not sexually appropriate with peers and staff. Dated 8/9/2011

He (Gage) had 3 incidents of sexually inappropriate behavior: 1 incident on 5/8/14 for playing with a hot dog at dinner referencing the sausage as his penis and the mayonnaise as semen, 1 incident on 5/12/14 for talking with a peer about boners, 1 incident on 5/7/14 for putting his hand in his pants to fondle himself while watching a female patient.
1 incident on 4/12/14 for putting his underwear over his pants and making sexual, suggestive gestures and thrusts with his body, 1 incident on 4/27/14 for inappropriately gesturing with a stuffed monkey as if the monkey was giving him
