REPORTER: The reporter/ Activities Director (Sharina) called with concerns for the victim, Evelyn. Evelyn has issues with vision, and it is unknown if she can perform her daily ADL’s. The reporter said the victim is not mentally capable of caring for herself. The reporter said the victim has no family. The victim home is in squalor conditions; there is clutter (garbage), and the kitchen and bathrooms are dirty. The reporter believes that the victim can not clean, and she may need to be connected to some resources that help assist her or be placed in a nursing home. This is the reporter’s 2nd time making the report for the victim. It is unknown when the reporter last saw the victim, but whenever the reporter goes to the home to inspect, the
