This will help the client answer efficiently. It makes the paperwork neat and easy to read. I also like that on the last page there is a section where the client has ample space to write anything else they want the therapist or social worker to know. This gives the client time to think about what they covered on the form so far and to really think about anything else that they believe is important. Also, if they feel like they didn’t have enough room to answer a particular section, they can go into more detail in this section.
Should the intake be re-formatted so it is easier to read? Why? No, I don’t think that it should be re-formatted. It is easy to read the way it is. Most of the questions have options to choose from, which makes it easier for the consumer to complete. I think that having options will help the consumer answer the questions more efficiently. For example, under the symptom section there is a list of options to choose from instead of a blank space. This can help the consumer identify symptoms that they may not have thought of in the first place. Furthermore, there are many yes or no questions and other questions that only require a check mark.
What would you include? What would you remove?
