Similar trends are found worldwide to mirror those of the United States. However, the progression of prostate cancer is typically slow and does not metastasize all that often. Due to this, treatments can typically be life saving and prostate cancer deaths have declined ~35% from 1997 to 2007. This can partially be explained by lifestyle changes, hormone therapy, radiation therapy, chemotherapy, and surgical treatment; however, large scale screening using prostate specific antigen (PSA) has to account for a substantial impact on the incidence and mortality rates for prostate cancer. …show more content…
Today, along with the DRE, PSA is the most common way physicians diagnose prostate cancer. However, PSA is not 100% accurate in diagnosing prostate cancer. It can be falsely elevated by certain conditions, both benign and malignant types of prostate cancer will elevate the PSA, and some men with prostate cancer will not have an elevated PSA. Even with these negatives the PSA has become the major serum marker for prostate cancer screening. Once an abnormal PSA comes back it is indicated for them to receive a prostate biopsy. So we will review the current guidelines for PSA screening and the current evidence both for and against

Also, studies identify limitations of PSA, including the low specificity as well as the low negative predictive value of the antigen (Klaus et al., 2014). The issue that remains is, because PSA is an organ-specific antigen, the PSA levels may be altered as a result of other factors such as ejaculation, the presence of inflammation, and as a result of benign prostatic hyperplasia (Mearini, 2013). Because PSA is not cancer-specific, it is difficult to determine a diagnosis based on elevated PSA levels alone, which often leads to additional unnecessary testing such as biopsies (Mearini,…

In the beginning of life, the prostate gland is approximately the size of a pea. The gland then grows throughout life, gradually until puberty is reached and then rapidly grows until the person reaches the third decade of their life. At this point, the prostate gland is considered to be the normal adult size. Benign prostatic hyperplasia (BPH) occurs when the prostate gland becomes larger than the normal adult size (Rodway & McCance, 2014). The enlargement occurs within the transition zone of the prostate by an uncontrolled cellular proliferation of the smooth muscle, glandular epithelium, and connective tissue (Patel & Parsons, 2014).…
