Primary Counselor met with Pt. for his monthly individual counseling session. Counselor and Pt. discussed his progress in recovery, his medication compliance and side effects, any personal concern and/or problems and current car accident. Pt. reported that he has been arguing with his girlfriend over money issues. Pt. indicated that his current prescribed medication is working well. Pt. is taking 8 mg of subutex and he is on a stable dose. Pt. denied having any medication side effects. Pt. requested to be present for Phase movement/increase TH on Wednesday. Pt. is concerned about the requirements that correspond with phase movement. Counselor explained that if he gets approved to move to Phase 5 he needs to attend this Clinic once a week to
