Issue/Problem Statement Missouri law should be changed to grant APRN’s full practice authority allowing more access to care, decreasing health care costs, and better patient outcomes in Missouri. Removing restrictions will also attract more nurse practitioners to practice in primary care in Missouri (Westat. 2015)
Stakeholders
As with any major change to policy, key stakeholders are noted for stating their position to the table. Changing the law to grant full practice rights to Missouri has several key stakeholders that have an agenda.
Key Stakeholders The stakeholder can be divided into those that want to give full practice rights to nurse practitioners and those stakeholders that want to keep the restrictions in place. Supporting the change for full practice authority are the Missouri Nurse Association, Missouri Association of Nurse Practitioners, Missouri Association of Rural Health Clinics, Missouri Coalition for Behavioral Health Care, Primary Care Association, Missouri Hospital Association, Missouri Family Health Council, and possibly certain hospital or nursing homes. On the other side of the aisle are those organizations that oppose full practice authority and keep current restriction in place. Stakeholders on full practice authority being given to nurse practitioners is divided into two different groups. The two groups are those that support and align with nurse practitioners and those that support and align with physicians. Nurse practitioners have the support of all nursing organizations, regardless of degree level or position. Advancement of nurse practitioners is seen as an advancement for nursing. Conversely, physician groups have rallied to support the cause of physicians and any deviation from current policy is seen as relinquishing control of the market and demand for

    Issues surrounding APN regulation include numerous titles and roles that confuse policymakers who determine reimbursement; state discrepancies in scope of practice and licensing criteria, creating hardships for prescriptive authority and reimbursement across state lines; and variation in composition of regulatory boards, ranging from pharmacy to medicine, among different states.9 Unfortunately, these challenges result in restricted scope of practice for NPs, limiting their ability to practice to the full extent of their education. For example, in California, collaborative agreements defining the parameters in which the APRN can perform delegated medical acts – disease diagnosis, treatment management, and medication prescription – are required for APRNs to practice. Reimbursement of APRN-provided services is another important issue that is determined by individual states.
