A reform system should reward value before volume, quality before quantity, and organized delivery over disorganized care. It is suggested that cost-sharing requirements and coverage should be a factor that encourages patients to utilize preventive services that could potentially delay or prevent onset chronic diseases.
Actions to improve healthcare quality should begin with Medicare payments as the initial impact geared toward implementing change in care which includes (1) changing the infrastructure of HIT (2) combining primary care and preventive care together (3) devising a new payment system to reduce cost growth and quality improvements. These changes along with the encouragement of Medicare, Medicaid and private payer’s cooperation, will develop accountability to help improve reforms.
