CMS refusal to pay for never misses cost has influenced policy and practice across the institutions. Preventive measures such as turning patients to avoid skin breakdown, assessment of patients at risk of fall and implementing appropriate measures, and use of incentive spirometry for postoperative patients are some the measures my institutions is really emphasizing. There are situations that Institute of Medicine recommended a favorable adjustment such as hospitals that receive complex and high risk patients who may develop complication even if the best care was offered (Mckeon, L., & Cardell, B. 2011).
