Zachary was transferred to a foster home in the Charleston Division on 7/28/16. MHP and Zachary completed exercises to assist Zachary’s ability to make healthy decisions. Zachary was able to complete simple tasks with prompts from MHP. Zachary’s behaviors improved based on foster parent reports of decreased redirection. Zachary entered foster care on 8/3/2015 with the Columbia Division. Zachary was transferred to the Charleston Division after a pre-adoption placement failed. Zachary has a history of disruptive, aggressive behaviors and refusal to comply with the rules of the home and community. Zachary behaviors improved based on reports by foster parents. Zachary was displaying minimal disruptive behaviors daily and complies with all the rules of the home. Zachary will benefit from positive reinforcement and redirection.
