Ms. T. A is a 42 year old African American female. The client is a veteran that is 100 percent service connected for paranoid schizophrenia. The client is currently single with two children. The client’s son is fourteen and she has a two year old daughter. The client presented for therapy related to her paranoid schizophrenia symptoms. Ms. T had paranoia symptoms associated with her diagnosis of paranoid schizophrenia. She was using a knife to cut up her clothes and furniture and then accusing her boyfriend and the maintenance worker at her apartment complex of destroying her property. T denied any substance abuse. She denied hallucinations and suicidal ideations. She reported that she had a ten pound weight loss. Her main support is her foster mother and cousin that is in the healthcare profession. Mr. T had a history of violent episodes such as confrontation with their brother and making threats about throwing her daughter out the window were the local police was notified. The client reported that her biological mother suffered from mental health disorders that led to her being placed in foster care. The client reported that she has recently regained custody of both of her children and that she has been taking her medications as prescribed. The date therapy began on was February 06, 2017. The end date for therapy is March 9, 2018. The number of sessions is 15. The client has attended all assigned sessions. Termination is planned based on the client’s demonstration of stabilization and symptoms are controlled with current medication regimen. In the acute phase of treatment the focus was to control the client’s symptoms related to paranoid schizophrenia and to increase the client’s quality of life. Mr. T was very cooperative during her psychotherapy sessions. She reported concerns about the anxiety and fear related to losing her children again. The client acknowledged the violate episodes that had happened and expressed that her medication regimen has been helpful in stabilizing her mood. She reported that she has not been in any recent confrontations or in contact with the police. Mr. T agreed to the medication regimen of Cogentin to reduce extrapyramidal symptoms and Abilify Maintena for mood stabilization. The client stated that she takes her medications daily and that her medications has been effective in controlling her paranoid schizophrenia symptoms. Family psychotherapy sessions were conducted that included the client’s sister and cousin to provide therapeutic communication that would direct the client towards stabilization and symptom reduction. The treatment effectiveness was monitored by using the client’s narrative during sessions. As
