Pa appeared to be clean, and was appropriately dressed in street clothes laying down on the bed when SC entered the apt. Pa's apartment was very small, clean but is a little cluttered. SC asked about the utilities and Mirvat reported that all are in working order, heating, electric, and cooling systems (the light and heat were on at the time of SC visit). Pa's that she has been sick the last four weeks. SC asked if Pa went to ER or was admitted to the hospital and the DCW reported that Pa does not like to go to the hospital so she went to the clinic instead. Pa was dx with two type diabetes and was put on medication Metformin hcl 500 mg to control it by her PCP. Pa seemed to be more in favor modifying her diet than medication. Pa also, reported that she is being treated for UTI with medication. SC asked about Pa’s mobility and Pa experiencing SOB, pain when walking, unsteady gait due to rib injuries weight (overweight). This makes is difficult for Pa to transfer and walk without hands on help. The DCW and informal has been helping Pa around the house and meeting all of her ADL/IADL needs. Pa cognitive functioning seemed to be intact. Pa dtr reported limited socialization due to Pa's mobility challenges makes it difficult to go out and socialize in community. SC asked Pa what she likes to do for fun and socialization she stated like to go to the mall shop and me new people. SC informed Pa that she can do that by scheduling CCT ride and asked her DCW or informal to accompany for a few hours a week. Pa and informal said they both like that idea. No falls were reported. Pa's needs and goals remain unchanged and Pa reports that services continue to meet her needs. Pa is not requesting any increase in care plan. SC reviewed Pa's ISP. Pa current ISP is 5 hours x
