D-The patient arrived on time for counseling session. The patient reported, she's stable on her dose and deny the need for a dose increase or decrease when offered by this writer. According to the patient, she's scheduled for counseling at CHR on 08/18/2016 at 4pm- which she meets with her counselor bi-weekly. Denies any recent relapse of illicit drugs. This writer reviewed the patient's UDS result as it was positive; however, reviewing her UDS result from January to present, some of her results were positive and negative. According to the patient, she does not have an explanation and says, " I take my medication as prescribed and that's it, Charlene. I swear to GOD......Oh God,....am I not going to get my take home bottle." This writer informed the patient, this writer only clarity as to whether or not anything changed and also how she takes her medication daily, at which the patient explained. …show more content…
This writer updated the patient on a recent conversation with Nursing Director Chrystal about her take home bottle being reinstated and the Nursing Director support the reinstatement, which had the patient
