An example of how EBP is utilized in your current practice would be or recent initiative to reduce polypharmacy in our clients. Polypharmacy can be defined as medications that exceed medical necessity; that can play as significant contributor adverse outcomes in geriatric populations. We have taking aspects of the evidence based practice guidelines and applied it our nursing practice. We quarterly evaluate the necessity for each medication in our clients and do medication reconciliation. We also help our clients consolidate their care through a central primary care provider to help prevent over prescribing of medications.
