The Population (P), intervention (I), Comparison or control (C), Outcome (O), and Timeframe (T) format was utilized to develop the project question, which guided the search for literature (Riva et al., 2012). The PICOT question devised for this evidence-based project is:
For the hypertensive adult patients in the outpatient medical clinic (P), will the implementation of a HBPM and nurse-led educational follow-up program (I) compared to no educational component (C) increase patient adherence to prescribed life-style modification and improve patient outcomes (O) over an eight-week timeframe (T)?

In addition, the participants the blood pressure classification and the participants will be requested to hang the normal blood pressure classification leaflet in eye-catching places to always act as a reminder. During each follow-up calls, the patients will be encouraged to share their achievements as well as challenges with the nurse practitioner. The project leader will develop educational resources such as leaflets, which will be tailored to patient’s unique needs to increase HTN knowledge and self-efficacy. The program will also involve screening of heart disease risk factors, pre-test and post-tests for self-efficacy, self-care behavior, and blood pressure knowledge, and blood pressure measuring skills will be …show more content…
Consistent with the existing literature, patient knowledge of HTN will be determined using Hypertension knowledge questionnaire. The questionnaire was designed by the National Institutes of Health for determination of patient HTN knowledge and had an acceptable level of reliability. The original questionnaire was developed from the existing guidelines and information published by various bodies that have an interest in heart disease, including CDC, American Heart Association, and National Lung and Blood Institute (Martins, Gor, Teklehaimanot, & Norris, 2000). The 12-items questionnaire involved with true and false answers. The HTN knowledge questionnaire will be administered before and after the implementation of the education and HBPM program. The available body of literature broadly supported the use of an existing knowledge test uniquely in this project. The questionnaire was used in Viera et al. (2008) study and was modified without upsetting the validity and reliability of the questionnaire
