: Resident stated “I do not have any friends here. I wouldn’t remember their names anyway” and “I am not really a part of this community.” Patient feels that her daughter is her only support.
O: Resident does not interact with other patients during my shift. She does not go to many community activities. Resident did not talk to the other residents when she was involved in a group collaboration activity.
A: Impaired social interaction related to disturbed thought processes aeb apparent lack of interaction with other residents (even when in a collaborative setting), and patient stating “I don’t have any friends here”.
P: 1. Promote consistent social activities that do not involve many distractions, such as going to the café every day. This
